I think they're the same person. It's hinted at various times that they are. And Roger Ebert pointed out that the film's title is a singular noun, not a plural, implying further that there is one persona belonging to both women. I think the most moving scenes are towards the end, such as when Alma forces Elisabet to say the word "Nothing."
